Fill in the health declaration

To participate in traffic, you have to be physically and mentally healthy. This is not only important not only for yourself but also for others in traffic. The health declaration consists of several questions about your health that you need to answer.

You must complete your health declaration online via the CBR. The driving school cannot do this for you because it concerns privacy-sensitive information. you are obligated to complete your health declaration truthfully. Failure to do so may have severe consequences at a later stage.

Do you have doubts about your health or a question on the health declaration? If so, first consult your driving school or family doctor before submitting it. If you accidentally fill in a possible issue with your health, while you are actually in good health, this may cause a serious delay in your driving course.

If you do have a health issue, the CBR may first send you to a medical specialist for an assessment. This process can sometimes take months. Submitting your health declaration early is therefore very important.

4. Pay for the declaration with IDEAL.

Attention: the declaration has only been submitted once you have paid.

5. You will receive an e-mail notification when you have a message.

This may be:

  • the acknowledgement of receipt of the declaration;
  • an overview of your supplied answers;
  • the Certificate of Fitness (Verklaring van Geschiktheid);
  • or a request to contact a doctor.

6. Inform your driving school when you have received a Certificate of Fitness.

Questions in the health declaration

Below you can read the questions you will be asked filling out a health declaration to get a driving licence. You can fill in the real health declaration via the CBR website. Watch out for different websites that offer the health declaration, they usually charge way more!

1. Do you have a limitation that makes it hard for you to use your arm, hand or fingers properly?

Enter ‘yes’ if:

  • you have only limited strength in your arm, hand or fingers.
  • you cannot use your arm, hand or fingers at all (for example, due to paralysis).
  • you are missing all or part of an arm or hand.
  • you have a prosthesis.

2. Do you have a limitation that makes it hard for you to use your leg or foot properly?

Enter ‘yes’ if:

  • you have only limited strength in your leg or foot.
  • you cannot use your leg or foot at all (for example due to paralysis).
  • you are missing all or part of a leg or foot.
  • you have a prosthesis.
  • you use medical aids to walk, such as a walking stick or walker.

3. Do you have difficulty seeing things that are far away, even if you wear glasses or contact lenses?

Enter ‘no’ if you can see well with glasses or contact lenses. This applies to all types of contact lens.

4. Are you being, or have you ever been, treated by an ophthalmologist (oogarts) for something other than glasses or contact lenses?

Enter ‘yes’ if, for example, you have been treated for a cataract or have had laser surgery on your eyes. Enter ‘no’ if you have had eyelid surgery or if you have been given eye drops to correct myopia.

5. Are you being treated for diabetes (diabetes mellitus)?

This means type 1 or type 2 diabetes.

6. Do you have chronic kidney disease (loss of kidney function)?

Enter ‘yes’ if you are receiving dialysis.

7. Do you have a lung disorder and, if so, do you use oxygen therapy while driving?

Oxygen therapy means you use extra oxygen from an oxygen tank while driving.

8. Have you had a lung, kidney, liver, pancreas or heart transplant?

9. Do you have, or have you ever had, a heart or blood vessel disease?

Enter 'yes' if:

  • you have had a heart attack, angioplasty (stent placement) or open-heart surgery.
  • you have a pacemaker, ICD or ventricular assist device (VAD).
  • you have a blood vessel disorder, such as an abdominal aortic aneurysm (AAA).

10. Has a doctor diagnosed you with a disorder of the spinal cord, nervous system or brain? Examples include a stroke, Alzheimer’s disease, dementia, muscle disease, neuropathy, Parkinson’s disease or MS (multiple sclerosis).

Examples are:

  • a stroke, intracerebral hemorrhage, brain tumor, brain damage.
  • Parkinson’s disease, MS, ALS.
  • muscle disease.
  • Alzheimer’s disease or other forms of dementia.
  • spinal cord injury, nerve damage or neuropathy.

Also enter ‘yes’ if a doctor has diagnosed you with a disease of the brain or nervous system not listed here. You can indicate exactly what is wrong in your answers to the follow-up questions.

11. Have you ever had an epileptic seizure?

There are many types of epileptic seizure. The exact type is not important for this question. Enter ‘yes’ if you have ever had an epileptic seizure.

12. Has a doctor diagnosed you with narcolepsy or hyper somnolence?

13. Do you have to use a CPAP/BiPAP mask or an MRA device prescribed by your doctor due to sleep apnea?

  • Sleep apnea is also referred to as OSAS or OSA.
  • If your doctor has indicated that you do not need to use a CPAP/BiPAP mask or an MRA device, please answer “no”.
  • On some Health certificates, this question is phrased as: 'Has a doctor diagnosed you with sleep apnea?'

14. Have you suddenly fallen unconscious two or more times during the past three years, for example through fainting?

There are many reasons why you may suddenly fall unconscious. Enter ‘yes’ if you suddenly fell unconscious two or more times during the past three years, regardless of the reason.

15. Have you suffered from spells of dizziness during the past year due to Ménière’s disease?

Enter ‘no’ if the dizziness was caused by something other than Ménière’s disease.

16. Has a doctor or psychologist (psycholoog) diagnosed you with a psychological or psychiatric disorder?

Enter ‘yes’ if a general practitioner (huisarts), psychologist (psycholoog) or psychiatrist (psychiater) has diagnosed you with a psychiatric disorder. This includes depression, psychosis or an anxiety disorder.

17. Have you made excessive use of alcohol, drugs or other addictive substances (i.e. substance abuse) in the past five years?

Enter ‘yes’ if:

  • you find it difficult to live without alcohol or drugs.
  • you regularly drive under the influence of alcohol or drugs.
  • you have been treated for alcohol or drug abuse during the past five years.

18. Do you use medicines that greatly impair your driving skills? If you are unsure, visit CBR.nl/medicijnen or ask your prescribing doctor.

Examples are medicines such as sedatives, tranquilizers and strong painkillers that can make you sleepy or drowsy. Also included are medicines that have serious side effects. See www.cbr.nl/medicijnen.

19. Do you have other symptoms or disorders that may impair your ability to drive a motor vehicle? Please explain.

You can answer this question with ‘yes’ if:

  • you are making little progress while undergoing driving training.
  • you wish to inform the CBR that you are deaf or that you have an intellectual disability.
